The
online luncheon reservation form gives you the option to
register for the September Super Sign Up & Welcome Event, for
luncheons as they occur, or the option to register for all events
(sign up and 8 luncheons) at once. Super Sign Up is $12, cost
per luncheon is $20, or for all the luncheons and including the sign
up is $172, and the last option is to pay for the year (Oct to May)
for $160. Yearly one time payment options only available through
October. You must have the latest Acrobat Reader on your
computer in order for this form to be filled out properly. You
can get this reader for free and install it on your computer.

You
can fill out your reservation form online and pay using your
paypal account. OR You can print the reservation form
(print the online reservation form), and send it with your
check.
Please
include names and phone numbers for anyone attending with you.
Reservation checks and Pay Pal transactions must be received
no
later than noon on the Friday before each Luncheon.
Cancellations must be called in to Jackie NLT noon on the
Friday before luncheon date.
After you fill out the application, you can pay on Pay Pal.
